Community Health Plans

First Nation community health plans are key elements of the Tripartite First Nations Health Plan. FNHC will work to ensure that every First Nations community has a community health plan that matches the vision of health that the community has for its people. This ongoing portfolio is in the process of development. Through the Community Engagement Hubs and/or Community Developers, the FNHC will work to ensure that all First Nations communities have the capacity and support required to successfully develop and implement their health plans.

Community Health Planning is occuring on multiple levels. The First Nations Health Directors Association (FNHDA) is leading the work to assist Health Directors with community health plans and the FNHC and Caucuses / hubs lead work developing regional health priorities.
