First Nation community health plans are key elements of the Tripartite First Nations Health Plan. FNHC will work to ensure that every First Nations community has a community health plan that matches the vision of health that the community has for its people. This ongoing portfolio is in the process of development. Through the Community Engagement Hubs and/or Community Developers, the FNHC will work to ensure that all First Nations communities have the capacity and support required to successfully develop and implement their health plans.
The initial steps are:
• Conduct an environment scan on the requirement of community health plans, including communities and health authorities.
• A FNHC subcommittee is being developed whose mandate would be to develop a holistic and comprehensive community health plan template. The FNHC is looking for volunteers to sit on this committee and anyone interested is to contact the FNHC Office.
